Screening steps for repeat surrogates
1. Zoom meeting
You will be working with our Surrogate Resource Team again throughout screening. Your coordinator will have a short Zoom meeting with you, (partner, if applicable), to check in and discuss your upcoming journey.
2. New questionnaire
After the Zoom meeting, your coordinator will send you a link to a new questionnaire. Please fill this out completely so it accurately reflects your most up to date information.
3. Medical records
We will be assisting you in collecting your records. We have the previous files, but we need the IVF records, prenatal records, and delivery summary for surro baby. We also ask that you have an up-to-date annual exam, pap (if due) and complete a new OB Clearance Form.
4. Psychological evaluation
Once your records are approved by our outside medical professionals, you and (partner, if applicable) will have an updated psychological evaluation. We will do our best to have you see the same psychologist as last time, but this is not guaranteed.
5. Home visit
If you have moved, you will have a new home visit with a social worker.
6. Insurance
Everybody’s favorite! If you have an employer plan, individual plan through the marketplace, or are covered on your partner’s policy, please start looking for the most up to date Benefit Booklet/Explanation of Coverage.
